B.1 Level Of Service Examples

Some examples of LOS Agreements are presented in this Appendix. These examples are based on actual systems, but have been adapted and adjusted for use here. These examples are not meant to reflect items which must be on a level of service agreement, but rather to suggest items that might be included. Your level of service agreement should be tailored to your system, and, above all, should reflect realistic levels of service that you are currently able to provide and ways to measure them. A blank form for your use is included after the examples.

The last page of this appendix is an example of a form used by Johnson County Wastewater to track its performance in meeting its level of service goals.
