Operator Certification EPA Region 6 Testing Application

This application is used to apply to take an operator certification exam as part of the EPA Region 6 Tribal Drinking Water Operator Certification Program and must be filled out and returned each time an operator wants to test at a certification level that they have not previously tested for.
