Operator Certification EPA Region 6 Reciprocity Application

This application is used by drinking water operators that are certified under a different program to apply for certification reciprocity with the EPA Region 6 Tribal Drinking Water Operator Certification Program and must be filled out and returned so that a reciprocity determination may be made.
