Operator Certification EPA Region 6 Retesting Application

This application is used to apply to retake an operator certification exam as part of the EPA Region 6 Tribal Drinking Water Operator Certification Program and must be filled out and returned each time an operator wants to retest at a certification level that they have previously tested for.
