Consider the following scenario:<\/span><\/b>\u00a0<\/span><\/p>\n

\u00a0<\/span>Asset:<\/span><\/b>\u00a0Chlorine pump. System uses\u00a0hypochlorite\u00a0so liquid chlorine solution is pumped into the system for disinfection. The system has both spare parts and a spare pump.<\/span>\u00a0<\/span><\/p>\n

Service History:\u00a0<\/span><\/b>Chlorine residuals are checked three times a week in the system. Unless the residual is\u00a0very low, the chlorine pump is not checked at the same time. It is checked only once every 2 weeks to ensure it is running but\u00a0otherwise it\u00a0is ignored. It receives no maintenance and is in poor condition.\u00a0The chlorine pump has failed due to many factors 7 times in the past 10 years. When the pump fails, there is no chlorine going into the system. This fact may not be\u00a0determined\u00a0until the residual is checked or if it happens to be the day the pump is inspected.\u00a0Once the lack of chlorine is\u00a0observed,\u00a0the well must be shut off until a new pump is installed. Having the well off for\u00a0the\u00a0short time\u00a0it takes to replace the\u00a0chlorine pump\u00a0is not problematic\u00a0since\u00a0the customers can be served from the storage tank. The pump is showing signs of potential failure.<\/span>\u00a0<\/span><\/p>\n

Probability of Failure Ranking\u00a0(PoF): 4<\/span><\/b>\u00a0<\/span><\/p>\n

Using a\u00a0PoF\u00a0scale of 1-5, this chlorine pump receives a\u00a0PoF\u00a0score of four (4)\u00a0because\u00a0the\u00a0pump is in poor condition, is not\u00a0maintained, has failed many times,\u00a0and is showing signs of failure.<\/span>\u00a0<\/span><\/p>\n

Customers may be drinking water that is not disinfected for two to three days before discovery. The consequence is mitigated by the presence of a spare pump and spare parts. However, because the pump may\u00a0fail for\u00a0multiple days before the failure is known, the consequence is not\u00a0substantially reduced\u00a0by the spare parts and pump.<\/span>\u00a0<\/span><\/p>\n

Heavy\u00a0rain is\u00a0not frequent.<\/span>\u00a0<\/span><\/p>\n
